Please note that parent consent for a referral must be obtained from the referring school.
Verbal consent may be obtained when a referral is being made. The specific details of verbal consent must be recorded.
Full fees must be paid before young person starts the provision and a holding charge of 25% of the full fee will be charged at the point of submitting this form.
